Thank you for your interest in Northern California Healthcare Institute’s Online Nurse Assistant Training Program. In this packet are information you will need to be considered for enrollment in our program. Attached you will find:

⦁ Be a minimum of sixteen (16) years of age
⦁ A parent, legal guardian or spouse who is of legal age is required to sign the 
      enrollment agreement.
⦁ High school diploma or a GED equivalency, or proof of passing the 
     ATB (Abilities to Benefit Test) with a score of at least 97.
⦁ Demonstrate proficiency in using English (sixth grade English Language Proficiency 
     Standard)
⦁ Live Scan/Criminal Background Clearance
⦁ Physical Exam within the last 12 months
⦁ Immunization proof or titer results confirming:
     - Tetanus (within past 10 years)
     - MMR (Measles, Mumps, Rubella)
     - Proof of Absence of Tuberculosis (negative skin test or negative chest x-ray within 
        6 months of start of program)
     - Varicella (titer or proof of vaccination)
⦁ Covid-19 Proof of Vaccination
⦁ Covid-19 negative test results (test 3 days prior to 1st day of class)
⦁ PC or laptop with the latest Windows or IOS operating system, camera, and  
      microphone.
⦁ Watch with second hand

IMPORTANT: All required documents and fees must be completed at least 2 weeks before the 1st day of class to be considered enrolled in the program.  Late submissions of required documents and fees will not be accepted.
